Vietnam Savaşı gazisi John Rambo, Burma'daki olaylardan sonra merhum babasının Arizona'daki at çiftliğinde, eski arkadaşı Maria Beltran ve onun torunu Gabrielle ile birlikte yaşamaktadır. Aradan geçen on bir yılda travma sonrası stres bozukluğu ile baş etmeye çalışan Rambo'nun en büyük yardımcısı genç Gabrielle olmuştur. Lise mezuniyetinden sonra Gabrielle, anneannesinin ve Rambo'nun tüm itirazlarına rağmen biyolojik babasını bulmak için Mekiska'ya gider.
